Job Description

  • Accountable for the conversion of diagnoses and treatment procedures into codes using an International Classification of Diseases.
  • Assigns codes for diagnoses/procedures in proper sequence to ensure appropriate reimbursement.
  • Special focus on provider professional code assignment.
  • Responsible for abstracting pertinent data into the hospital information system to ensure correct billing and indexing.
  • Ensures coding of all diagnoses/procedures is in accordance with ICD-10-CM/CPT-4 coding principles/guidelines.
  • Is knowledgeable of all charting and coding requirements for each service type being coded.
  • Utilizes computerized coding and abstracting equipment.
  • Reviews medical records thoroughly to ascertain all diagnoses/procedures are coded.
  • Maintains an acceptable level of coding accuracy with minimal denials.
  • Refers encounter to Coding Team Lead or HIM Director if question regarding diagnoses/codes.
  • Additional assignments, as needed.

Qualifications

  • High school graduate or equivalent
  • Graduate of an approved course in medical terminology and anatomy/physiology
  • RHIT, CCS, CCS-P accreditation required
  • Proficient in ICD-10-CM and CPT coding principles
  • A minimum of two years experience as a coder in an acute care facility preferred
  • Ability to effectively communicate in English, both verbally and in writing
  • Basic computer knowledge
